The assessment took place on the 23 September 2025. The last rating for this service was requires improvement (published 24 September 2024). During the last assessment we identified breaches of legal regulations relating to consent, safe care and treatment and good governance. This assessment was planned to ensure the provider had made the required improvements. We found they were no longer in breach of legal regulations and the new rating for the service is good. City Home Care Limited is a domiciliary care agency. It provides personal care to people living in their own homes. The service is also registered to provide supported living services. At the time of our assessment, the provider was not operating any supported living services. The service is registered to provide care to children and adults, including people with a learning disability. At the time of our assessment, 52 adults were using the service. The agency was not providing care to children. Most people were older adults and adults with physical disabilities. We expect health and social care providers to guarantee people with a learning disability and autistic people respect, equality, dignity, choices and independence and good access to local communities that most people take for granted. ‘Right support, right care, right culture’ is the guidance CQC follows to make assessments and judgements about services supporting people with a learning disability and autistic people and providers must have regard to it. The provider was meeting the principles of Right support, right care, right culture. People received safe care. Staff assessed risks to people’s safety and planned for these. People were supported to take their medicines in a safe way. There were enough staff to care for people. Staff arrived on time for care visits and carried out the allocated tasks to meet people’s needs. Staff were well supported. They had guidance and training to help them with their jobs. Staff communicated well with each other and external professionals. People’s healthcare needs were assessed and planned for. Staff monitored people’s wellbeing and reported any concerns. People were asked to consent to their care. Staff treated people with kindness. People were able to make choices about their care and support. People were involved in planning their own care and contributing to reviews. People were supported to be independent and do things for themselves when they were able to. The provider had effective systems for monitoring and improving the quality of the service. These included investigating complaints, accidents and incidents, carrying out a range of audits and asking stakeholders for feedback.

We undertook this assessment to follow up on concerns identified by the local authority. These included staff not being trained, supervised appropriately, staff being late for visits, risk assessments not being carried out appropriately and incomplete records. We did not assess all quality statements at this assessment. For those areas we did not assess, we used the ratings awarded at the last inspection to calculate the overall rating. At this assessment we identified improvements were needed. We found three breaches of regulations regarding safe care, consent to care and good governance. Appropriate risk assessments were not always in place and the provider did not always obtain consent from the person to provide them with care. The provider had developed care plans for each person, but these did not always contain the most up to date or detailed information regarding the support the person required. We will request an action plan from the provider to understand what they will do to improve the standards of quality and safety.
