This focused inspection of Rainbow Medical Services Ltd, covering Safe and Well-Led only, found the service rated Good in both domains following previous concerns about management competency and staff punctuality. Rainbow Medical Services Ltd was rated Good across all five key questions at its first comprehensive inspection under the current registration on 16 August 2017. Minor areas for improvement were identified in risk assessment recording and complaints record-keeping, and the service was operating without a registered manager, though two managers were in the process of registering with CQC.

· Robust recruitment processes introduced to select staff with the right attributes, significantly reducing staff misconduct incidents.
· Comprehensive induction, training, supervision (4 sessions/year), and annual appraisal programme for all staff including specialist clinical training.
· Effective medicines management with newly appointed clinical manager improving competency assessment of staff.
· Strong risk management plans in place for complex clinical needs including tracheostomy, catheter care and pressure ulcer prevention.
· Excellent staff recognition and reward systems including annual award ceremony, pay review, and benefits package, boosting staff motivation.

This targeted inspection of Rainbow Medical Services Ltd focused solely on infection prevention and control amid COVID-19 concerns, finding strong PPE compliance and testing arrangements but identifying a gap in documented risk assessments for BAME and other high-risk staff. · IPC and PPE policies were regularly updated to reflect COVID-19 guidance changes, with contingency plans for outbreaks and staff shortages.
· Infection risks to people receiving personal care were thoroughly assessed, including dedicated support bubbles and shielding arrangements for high-risk staff.
· The service had adequate PPE supplies to meet current demand and foreseen outbreaks.
