Date of assessment: 19 January 2026 The service is a care at home service providing support to people living with disabilities and mental health needs. This was the first assessment of the service. The service demonstrated a strong commitment to safety and continuous learning. Staff were trained to manage risks and learning from incidents informed improvements. Safe processes supported admissions, transitions, and referrals, with assessments completed before people moved in and updated after hospital stays or health changes. Safeguarding procedures were effective, environmental risks were managed, and people were supported to understand risks using accessible information. Staffing levels met assessed needs, recruitment was safe, and infection‑control measures followed best‑practice guidance. Medicines, were managed safely in line with STOMP principles, with GP and community‑team input. Assessments were holistic and person‑centred with external professionals contributing to planning and reviews. People accessed timely healthcare, and staff followed the Mental Capacity Act, seeking consent and involving families and advocates in best‑interest decisions. Care promoted dignity, independence, and personal choice in line with Right Care, Right Support, Right Culture, and staff responded promptly to subtle changes in health or behaviour. The service was flexible, inclusive, and responsive, with people and families actively involved in decisions. Visible leadership, strong governance, and effective quality assurance promoted safety, continuous improvement, and coordinated, holistic care supported by external partnerships and a commitment to learning and innovation.
