Dates of assessment: 3 November 2025 to 14 November 2025. We visited the registered office and people using the service in their homes during this assessment. This was a responsive assessment in response to concerns about people’s safe care and treatment. The provider runs a supported living service. At the time of our inspection there were 7 people using the service and all received the regulated activity. We found people’s needs were not always effectively assessed and managed. Staff had not received all the appropriate training to safely support people with specific needs, and there was a lack of detail and evidence of effective risk management. We found the provider did not always train and assess their staff appropriately before providing support to people. The application of the Mental Capacity Act was not in place, with missing assessments and restrictive practices not always considered in the context of best interests. Care plans did not consistently reflect people's goals or aspirations. The oversight of the service was not robust. Although there was a registered manager, and an additional site manager; they were not always able to demonstrate knowledge about all 7 service users to ensure adequate oversight and governance of the service. The provider was found to be in breach of five legal regulations. This related to consent, safe care and treatment, safeguarding, good governance and staffing.
