Date of inspection: 2 to 13 February 2026. Main Site, known as Blue Arrow Healthcare, is a care at home service providing support to children and adults, including people who have sensory impairments, learning disabilities and autistic people. At the time of our inspection the service was being provided to 10 people, who lived in Leeds and Wakefield. Not everyone who used the service received personal care. The Care Quality Commission (CQC) only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do; we also consider any wider social care provided. At the time of this inspection, 3 people, including children, were receiving support with personal care. We assessed the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed autistic people and people with a learning disability respect, equality, dignity, choices, independence and good access to local communities that most people take for granted. At the time of the inspection, the service was supporting people with a learning disability. The provider showed a broad understanding of how to apply the ‘Right support, right care, right culture’ guidance but improvements in its application were required. This was the first inspection of this service since registration. We reviewed all quality statements. At this inspection the provider has been rated requires improvement. The service was in breach of the regulation linked to governance. We found some aspects of the management and oversight at the service were not effective. The provider failed to implement effective processes to monitor and improve the quality of the service and identify the issues found during our inspection. Records were not always complete.[AB1] The management of medicines was not always managed safely. Risk assessments were not always in place or were not detailed enough. This included a lack of environmental risk assessments or assessments linked to people's care. We discussed this issue with the provider, and after our visit we reviewed evidence confirming risk assessments had been reviewed or put in place. Most aspects of recruitment were completed in a safe way. Most staff had received mandatory training, and staff had their competencies assessed. However, records linked to training and competencies were not always kept up to date. Staff did not always receive appraisals in line with the provider’s policy. This had been identified by the provider and a plan put in place to address this. Staff told us they felt supported by the management team. People received care that was centred around their needs. Staff knew about people's needs. However, we found care records were not always consistent or complete. We received positive feedback from people and relatives about the management team. The registered manager was responsive to this inspection’s findings and feedback, and took action, or told us the action they would take to address the issues identified.
