Date of assessment 15 October to 13 November 2025 with on-site visits taking place on 15 October and 16 October 2025. Rialto Care & Support Ltd provides care and support to people living in supported living settings, so they can live as independently as possible. This specialist service is used by autistic people or people with a learning disability, as well as those living with physical disabilities or sensory needs. We assessed the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed people with a learning disability and autistic people respect, equality, dignity, choices, independence and good access to local communities that most people take for granted. At the time of the assessment, 8 people received the regulated activity of personal care. This was an unannounced responsive assessment due to concerns reported to the Care Quality Commission. During the assessment we looked at all 5 key questions. People were empowered to be independent either with or without the support of staff. Feedback from relatives and health professionals corroborated this and was positive. The provider worked well with other health professionals to support people to live as independently as possible and people were actively integrated into the community. We observed staff treating people with dignity and respect. Staff followed care plans which supported positive outcomes for people using the service. Staff were safely recruited and completed appropriate training including bespoke training tailored to people’s needs. Additional training had been scheduled by the provider to meet people’s current needs. Staff received competency checks and regular supervision. Care plans and risk assessments were updated in line with the changing needs of people to guide staff on how to support people appropriately, however, information was sometimes conflicting or missing. Some governance systems failed to identify these concerns and improvements were required around the storage and disposal of medication, however, a new system had been implemented to remedy this. Both relatives and professional partners stated staff knew people well, met people’s needs and thought people were well cared for and safe. ADDING ADDITIONAL CONTENT
