Date of Assessment: 04 to 29 September 2025. The service is a care at home service providing support to older and younger people living in their own home living with dementia, mental health conditions, physical disabilities or sensory impairments. The service had 2 breaches of regulation from the June 2024 assessment in relation to staffing and fit and proper persons employed. The service had 6 breaches of regulation identified at the December 2024 assessment in relation to good governance, receiving and acting on complaints, statement of purpose, fees, display of ratings and notification of other incidents. We issued a warning notice after the December 2024 assessment in relation to good governance. We followed this up in March 2025, the provider had made improvements but continued to be in breach of good governance. We carried out this assessment to review actions taken by the provider following our last assessment. We assessed 14 quality statements in safe, responsive and well led. At this assessment we found the provider had made improvements and was no longer in breach of regulations relating to staffing, fit and proper persons employed, good governance, receiving and acting on complaints, statement of purpose, fees, display of ratings and notification of other incidents. The overall rating of the service has changed to good. Improvements had been made to address previous areas of non-compliance. Accidents, incidents and safeguarding concerns were now well managed and actions taken to prevent future occurrence. Key organisational documents such as the statement of purpose and policies and procedures had been updated. People’s care plans were regularly reviewed and people were involved in this process. The provider ensured complaints were managed effectively and were open and transparent when things had gone wrong. Learning was now shared and communication with staff and people was effective. The provider had displayed its Care Quality Commission (CQC) assessment rating at the location. Governance systems had been revised but further improvement was needed to ensure they were fully robust. We found improvements in recruitment and training had been made but governance systems did not ensure gaps were identified in a timely way or the quality of training always met staff’s needs. The provider did not always evidence areas which required further development to ensure continual improvement. Staff and professionals told us the service had improved. The manager was praised for their engagement with people and ensuring people and staff were listened to.

The overall rating of the service has remained requires improvement. Key documents such as the statement of purpose and service user guide had been reviewed and updated. Clearer management oversight of safeguarding, complaints and accidents and incidents was evident. The provider was developing audit documents to monitor the quality and safety of the service. However, there was no service improvement plan. We have asked the provider for an action plan in response to the continued breach of regulation found at this assessment.

This was a responsive assessment based on information we received about the management of complaints and the providers actions to our last assessment. We assessed 1 quality statement in safe, 2 in responsive and all the quality statements in well led. We found 6 breaches of the legal regulations in relation to receiving and acting on complaints, good governance, statement of purpose, fees, display of ratings and notification of other incidents. The service had 2 breaches of regulation from the June 2024 assessment in relation to staffing and fit and proper persons employed. The service lacked governance systems to ensure it delivered high quality, safe, person-centred care. Key organisational documents such as the statement of purpose and policies and procedures were not up to date or reviewed regularly. People could not be assured complaints were managed effectively. The service’s processes did not drive improvement and learning. People said their care experiences had improved since the last assessment. Staff told us of positive changes since a recent change of management. In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/ or appeals have been concluded. We have asked the provider for an action plan in response to some of the concerns found at this assessment.

We spoke with people who use the service, their relatives and staff between 20 June to 25 June 2024. Seraphim Home Care is a domiciliary care agency (DCA), providing personal care for people living in their own homes in the community. This was a scheduled assessment which was brought forward due to concerns raised to us about the service. This is the first assessment of the service. At the time of our assessment there were 15 people using the service. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided. We found 2 breaches of the legal regulations in relation to staffing and fit and proper persons employed. Staff had not always been recruited safely or trained sufficiently to meet people’s needs. This placed people at potential risk of harm. We assessed a small number of quality statements from the safe, caring and responsive key questions and found areas of concern. The scores for these areas have been combined with scores based on the key question ratings from the last inspection. Though the assessment of these five quality statements indicated areas of concern since the last inspection, our overall rating remains good. We have asked the provider for an action plan in response to the breaches of regulation found at this assessment.

Seraphim Home Care received a Good rating across all five key questions at its first CQC inspection in April–May 2015, with 19 people receiving personal care. The service demonstrated safe staffing, robust recruitment, effective training and supervision, personalised care planning, and strong leadership with no failures or regulatory breaches identified.
